Once the form downloads and you open the document, be sure to select the "Enable Editing" which will allow you it fill it out. The document is not linked to inventory. It is simply designed for you to download fill out and save prior to emailing or printing then mail or fax.   

When you are done, Save the document as a .PDF and send it as an attachment to email address: orders@fra.org or you can print out the form and mail or FAX it to HQ.
